According to the World Health Organization, healthier environments could prevent nearly one quarter of the global burden of disease. That makes environmental quality not just an ecological concern – it is a public health priority. Understanding what environmental quality means, why it matters, and how we can improve it is essential for building healthier communities and a more resilient planet.

What is environmental quality?
Environmental quality refers to the overall state of our natural and built surroundings and how they influence human health, wellbeing, and ecosystem function. It encompasses the condition of the air, water, land, and the human-made structures around us. When these elements are clean, safe, and well-maintained, both people and ecosystems flourish. When they are degraded – through pollution, habitat destruction, or poor infrastructure – health outcomes suffer.
The U.S. Environmental Protection Agency (EPA) developed the Environmental Quality Index (EQI), which measures environmental quality across five domains: air, water, land, built environment, and sociodemographic environment. This tool helps researchers understand how cumulative environmental exposures – not just isolated pollutants – relate to health outcomes such as cancer incidence, asthma prevalence, infant mortality, and obesity. The key insight here is that environmental quality is not about one single factor. It is the combined effect of multiple environmental conditions acting together on human health.
Healthy People 2030, a U.S. federal initiative, identifies environmental conditions as a key social determinant of health. It notes that water quality, air quality, and weather patterns vary significantly among different populations and geographic locations, and these variations are often shaped by historical, economic, and sociopolitical factors. This means environmental quality is also an equity issue – low-income communities and communities of colour often bear a disproportionate burden of environmental degradation.
Natural vs. built environments
Environmental quality depends on the interplay between natural environments – forests, wetlands, rivers, and open green spaces – and built environments – cities, roads, factories, and housing. Both have a profound effect on health, and the challenge lies in balancing development with ecological preservation.
The role of natural environments
Natural environments provide what scientists call ecosystem services – benefits that humans derive from nature, often without even realising it. Forests filter the air. Wetlands purify water. Green spaces reduce stress and improve mental health. Research consistently links access to natural areas with better respiratory health, lower rates of cardiovascular disease, and improved psychological wellbeing.
However, when natural habitats are destroyed or degraded through urbanisation, deforestation, or industrial pollution, these benefits disappear. Air quality declines, water contamination increases, and communities become more vulnerable to flooding and extreme weather events.
The impact of built environments
The built environment – where most people now live and work – creates its own set of environmental quality challenges. Poorly planned cities lead to traffic congestion, which increases air pollution. Inadequate waste management systems create breeding grounds for disease. Lack of green spaces in urban areas contributes to the urban heat island effect, where cities become significantly warmer than surrounding rural areas.
A study published in PMC highlights that automobile-centric urban designs – common in post-World War II development – have created built environments that discourage physical activity and increase exposure to vehicular air pollution. These design choices have measurable impacts on chronic disease rates, including heart disease, obesity, and respiratory illness.
Pollution control and waste management
Controlling pollution and managing waste effectively are two of the most critical steps in maintaining environmental quality in both natural and built settings. Air pollution alone is responsible for millions of deaths annually, contributing to conditions ranging from lung cancer to stroke. Indoor air quality is equally concerning – household air pollution from cooking fuels, mould, and chemical products contributes to nearly 4.3 million deaths per year globally.
Water pollution presents another major threat. Healthy People 2030 emphasises the need to increase the proportion of people whose water supply meets Safe Drinking Water Act regulations. Contaminated water – whether from industrial runoff, agricultural chemicals, or ageing infrastructure – causes gastrointestinal illnesses, developmental problems in children, and long-term chronic conditions.
Effective waste management systems, including proper hazardous waste disposal, recycling programmes, and sewage treatment, are foundational to preventing environmental degradation. Without them, toxic substances leach into soil and groundwater, creating long-term health risks for entire communities.
Protecting natural resources
Preserving high-quality natural areas is not just about conservation for its own sake – it is a public health strategy. Ecosystems like wetlands, watersheds, and forests serve as natural infrastructure that protects communities from environmental hazards while providing essential resources like clean water and clean air.
Why wetlands matter
Wetlands are among the most valuable ecosystems on the planet, yet they are often underappreciated. The EPA notes that wetlands serve multiple critical functions: they control erosion, limit flooding, moderate groundwater levels, filter nutrients from runoff, and protect drinking water sources. A single acre of wetland can store approximately one million gallons of water, making wetlands powerful natural buffers against floods and storms.
NOAA Fisheries reports that coastal wetlands save vulnerable communities an estimated $23 billion each year by reducing flood damage. During Hurricane Sandy, for example, wetlands shielded parts of the East Coast from over $625 million in direct flood losses. Wetlands also act as carbon sinks, absorbing greenhouse gases like carbon dioxide and storing them in vegetation and soil – a critical function in the fight against climate change.
Despite these benefits, the National Wildlife Federation reports that more than half of the wetlands in the lower 48 U.S. states have been lost since colonisation. Ongoing threats from development, sea level rise, and agricultural runoff continue to put remaining wetlands at risk.
Watershed protection
A watershed is the entire area of land that drains into a particular water body – a river, lake, or ocean. Everything that happens within a watershed affects the quality of the water downstream. Agricultural fertilisers, pesticides, sewage, and urban runoff can all make their way into watersheds, degrading water quality and harming aquatic ecosystems.
The USDA’s Natural Resources Conservation Service (NRCS) provides technical and financial assistance for watershed conservation. Their programmes help prevent erosion, restore damaged infrastructure, improve water and air quality, increase filtration, and enhance wildlife habitat. These efforts span from headwaters to coastlines, addressing the unique concerns of each region.
Strategies for natural resource protection
The EPA recommends three key strategies for maintaining the environmental benefits that wetlands and other natural areas provide:
Preservation focuses on preventing development activity in sensitive natural areas while encouraging responsible land management upstream. This includes buffer zones around wetlands and zoning regulations that limit construction in flood-prone or ecologically significant areas.
Restoration involves rehabilitating degraded ecosystems to their former function. This means replanting native species, removing invasive plants, and using structural devices to restore natural water flows. Volunteer programmes through organisations like the National Park Service actively engage communities in wetland and watershed restoration by removing invasive species, planting native vegetation, and rebuilding natural landscape features.
Constructed treatment systems use engineered wetlands and vegetated filter strips to pretreat stormwater runoff before it reaches natural water bodies. These systems can achieve sediment removal rates above 90 percent and are often less expensive to build and maintain than traditional stormwater treatment infrastructure.
Community involvement in environmental quality
Governments and organisations can set policies and provide funding, but lasting improvements in environmental quality require active participation from local communities. People who live in a place understand its challenges better than anyone, and their engagement ensures that environmental initiatives are relevant, effective, and sustainable over time.
Why community participation matters
When community members are involved in environmental decision-making, the results tend to be more equitable and longer-lasting. Environmental justice – the equitable distribution of environmental risks and benefits – depends on meaningful community involvement. Without it, decisions about where to place polluting industries, landfills, or waste treatment plants tend to disproportionately affect marginalised communities.
The case of Flint, Michigan, is a well-known example. Residents lived through a water crisis caused by lead contamination of the city’s drinking supply, and community members were largely excluded from the decision-making that led to the problem. It took years to resolve, affecting over 100,000 people. This illustrates what happens when communities are left out of environmental governance.
Sustainable practices at the local level
Communities can drive environmental quality improvements through a range of practical initiatives. Recycling and waste segregation programmes, such as the decentralised waste collection model in Pune, India, demonstrate the power of grassroots action. That programme employs over 3,000 waste workers and diverts approximately 50 percent of the city’s waste from landfills through source segregation, recycling, and composting.
Other effective community-level practices include:
Urban gardening and green space creation – community gardens improve local air quality, reduce urban heat, provide food security, and strengthen social bonds among residents.
Water conservation initiatives – local programmes that promote rainwater harvesting, efficient irrigation, and reduced water waste help protect both groundwater supplies and downstream water quality.
Clean-up drives and habitat restoration – organised events where residents remove litter from waterways, plant native species, or restore degraded habitats create immediate environmental improvements while building long-term environmental awareness.
Energy conservation campaigns – simple actions like switching to energy-efficient appliances, reducing electricity use, and adopting renewable energy sources at the household level collectively make a significant difference in reducing pollution and greenhouse gas emissions.
Awareness and education programmes
Awareness is the foundation of community involvement. People cannot act on what they do not understand. Environmental education programmes in schools, community workshops, and public campaigns play a critical role in helping residents understand how their daily choices – from waste disposal to energy use to transportation – affect environmental quality.
Citizen science projects, where community members help collect environmental data such as water quality measurements or air pollution readings, serve a dual purpose. They generate valuable scientific data while also deepening participants’ understanding of and connection to their local environment. A 2024 paper in Frontiers in Public Health emphasises that well-prepared practitioners from all walks of life – not just scientists – are needed to respond effectively to complex environmental health challenges. Training and capacity building at the community level is essential for strengthening society’s overall ability to protect environmental quality.
The link between environmental quality and health outcomes
The connection between the quality of our environment and our health is supported by a growing body of evidence. A 2024 scoping review in Healthcare analysed 166 studies and found strong associations between environmental factors – air pollution, climate change, solar radiation, and weather patterns – and a wide range of health conditions, including respiratory and cardiovascular diseases, neurological disorders, cancers, and infectious diseases. The review underscored that people are exposed to multiple environmental stressors simultaneously, and these interactions compound health risks.
The WHO estimates that in 2016, approximately 13.7 million deaths globally – about 24 percent of all deaths – were linked to modifiable environmental risk factors. Noncommunicable diseases like heart disease, chronic respiratory conditions, and cancers bear the heaviest burden. Air pollution alone is a leading contributor. But the picture extends beyond air quality. Contaminated water, hazardous chemical exposures, unsafe workplaces, and poorly designed urban environments all play a role.
This evidence makes one thing clear: improving environmental quality is one of the most effective forms of preventive healthcare available. Investments in cleaner air, safer water, healthier built environments, and protected natural spaces pay dividends not just in ecological terms, but in reduced disease burden, lower healthcare costs, and longer, healthier lives.
Moving forward
Improving environmental quality is not the responsibility of any single entity – it requires coordinated action across governments, industries, scientific institutions, and communities. Policies like the Clean Air Act and Safe Drinking Water Act have proven that regulatory frameworks can significantly reduce pollution and its health effects. But regulations alone are not enough. Natural resource protection, sustainable urban planning, effective waste management, and active community engagement must all work together.
The good news is that solutions exist and many are already being implemented around the world. From wetland restoration programmes to community-led recycling initiatives to urban greening projects, there are proven pathways to better environmental quality. The challenge is scaling these efforts and ensuring that the benefits reach all communities – especially those that have historically borne the greatest environmental burden.
